Pharmacist (WEST) | Healthcare | Monday to Friday office hour Location: West Working Hours: 5 Days (Monday – Friday), 42 hours per week Job Responsibilities • Provide pharmaceutical care as part of a multidisciplinary healthcare team under the NUHS@Home programme. • Review patients' transfer medications and contact patients prior to transfer to assess medication supply requirements. • Conduct medication counselling through face-to-face and virtual consultations. • Perform medication reconciliation to ensure accuracy and continuity of care. • Review discharge prescriptions and verify new medication orders from physicians. • Identify and flag inappropriate medications or dosages where necessary. • Assist with medication supply duties and monitor medication preparation status. • Handle other duties as assigned based on operational requirements. 📩 Interested candidates, please send your updated CV to jocelynchan@recruitexpress.com.sg or Telegram @jocelynchan.
