Pharmacist (Acute and Community Hospital, Inpatient Dispensing, Clinical Review, Shift Work) #HDC1

A new public hospital is looking for Pharmacists to provide safe, effective medication services across acute and community care. You will review prescriptions, work with clinicians and support patients on their therapy. What You'll Be Doing • Review each patient's medication chart or prescription for appropriateness and contact prescribers when intervention is needed • Check that orders are clear, doses are within therapeutic limits and potential interactions are identified • Fulfil drug orders accurately and in a timely manner • Dispense medications and explain indications and administration to patients • Provide drug information and teaching to other healthcare professionals • Ensure costs and revenue related to medications are accounted for and recovered accurately • Work with doctors, nurses and allied health in multidisciplinary care • Support medication safety, audits and quality improvement initiatives • Maintain accurate documentation and records • Work rostered shifts, including overnight, as scheduled What You'll Bring • Degree in Pharmacy and above, with registration with the Singapore Pharmacy Council • Experience in acute or community hospital pharmacy is an advantage • Strong knowledge of pharmacotherapy and medication safety • Sound clinical judgement and attention to detail • Strong stakeholder engagement skills • Able to work as part of a multidisciplinary team • Willing to work rostered shifts including overnight • Committed to continuous learning What's In It For You • Shift allowance where applicable • Exposure to integrated acute and community care • Public healthcare environment Interested?
