6723 - Community Social Worker

Working location: Tampines Working hours: 5 days(Mon – Fri), 9 – 6pm Salary: $4,700 - $5,300 Responsibilities: • Initiate and/or participate in outreach efforts related to the Division, which include developing outreach plans to publicize the Division Programmes and establishing good relationships and contacts with stakeholders or community partners. • Management of referrals, and engaging referral agency / person for intake and assessment. • Case assessment and conceptualization by conducting a thorough assessment of the case using information gathered during the intake and subsequent engagement with client, family and stakeholder. • Collaborate with clients to develop intervention plans and provide counselling and support to address their risks and needs issues; advocate for clients' rights and access to essential services within the community and relevant systems. • Documentation of all case related records. • Research and evaluation through keeping abreast of current research and trends and participate in and support research projects. • Professional development and education by preparing for, attending and presenting cases at Internal Case Reviews / Case Conceptualization meetings / Clinical supervision on a regular basis. Involve in supervising interns and/or students on placement • Develop and implement new projects/initiatives that are aligned with the objectives of the Division.
