Permanent Staff Nurse | Geriatric & Dementia Care #HYN

About the Role We are seeking a dedicated and patient-oriented Registered Nurse to join a Senior Day Care Centre, providing quality nursing care and support to elderly clients, including those with dementia. This is a great opportunity for nurses with acute hospital experience who are keen to broaden their clinical exposure and transition into community and geriatric care. Job Responsibilities • Assess clients' nursing needs and provide quality geriatric and dementia care. • Conduct pre-admission assessments for new clients and facilitate their enrolment in accordance with established procedures. • Develop, implement and evaluate individualised nursing care plans in collaboration with the multidisciplinary healthcare team. • Monitor clients' health conditions and progress, ensuring appropriate follow-up and intervention. • Perform nursing procedures including medication administration, wound dressing and other clinical duties. • Provide health education, counselling and guidance to clients and their family members. • Maintain accurate nursing records and ensure timely submission of relevant clinical documentation. • Supervise and guide nursing and care staff to maintain appropriate clinical standards and work competency. • Monitor clinical and service quality indicators and participate in continuous improvement initiatives. • Respond to medical emergencies and provide CPR, AED and first aid when required. • Support health promotion programmes, infection control campaigns and clinical service projects. • Participate in staff training, professional development and other duties assigned by the management. Requirements • Registered Nurse with a valid Singapore Nursing Board (SNB) practising certificate. • Ideally 2–3 years of permanent acute hospital nursing experience. • Candidates with a Certificate in Geriatric Nursing or relevant qualifications will have an added advantage. • Open to candidates without prior geriatric experience. • Good communication and interpersonal skills. • Responsible, patient and committed to delivering quality nursing care. • Must be comfortable working 4 weekdays and 1 Sunday per week, from 8.30am to 6.00pm.
