Physiotherapist (Acute and Community Hospital, Inpatient and Outpatient Rehabilitation, Patient-Centred Care) #HDC1

A new public hospital is looking for a Physiotherapist to deliver rehabilitation across acute and community settings. You will help patients recover, regain function and live independently. What You'll Be Doing • Assess patients and develop individual physiotherapy treatment plans • Deliver inpatient and outpatient rehabilitation across acute and community care • Set measurable goals and monitor patient progress • Educate patients and caregivers on exercises, self-management and safe mobility • Work with doctors, nurses and allied health in multidisciplinary care • Document assessments, treatments and outcomes accurately • Support discharge planning and transitions of care • Apply evidence-based practice and contribute to service improvement • Participate in clinical audits, training and continuing education • Support team members through mentoring and knowledge sharing What You'll Bring • Degree in Physiotherapy and above, with valid registration with the Allied Health Professions Council • Experience in acute or community hospital settings is an advantage • Sound clinical reasoning and assessment skills • Passion for patient-centred, evidence-based care • Strong teamwork and collaboration skills • Strong stakeholder engagement skills • Able to work as part of a multidisciplinary team • Committed to continuous learning What's In It For You • Structured professional development • Exposure to integrated acute and community care • Public healthcare environment Interested?
