You will serve as Licensed Practical Nurse with the Patient Aligned Care Team (PACT) for the Nursing Services with the Community-Based Outpatient Clinic located in Spokane Valley, Washington.
PRINCIPALES MISSIONS :
As part of a Patient Aligned Care Team (PACT), you'll work alongside a dedicated group of professionals, including a Primary Care Provider, a Nurse Care Manager and a Medical Support Assistant to deliver team-based, veteran-centered care. PACT focuses on whole-person care through ongoing relationships, preventative health, chronic disease management and care coordination. Room Veterans in a timely manner. Provide care within their scope of practice, including any recommendations/vaccinations and WH's-specific care. Use formal and informal communications that are respectful, effective, timely, and bidirectional with all team members to convey significant, clinically relevant information for the care of Veterans (includes instant messaging with team members, messaging in the electronic health record, and e-mail). Assist provider with minor general and Women's Health-specific procedures. Review provider's weekly/daily schedule to address any scheduling discrepancies. Participate in team performance improvement and sustainment activities to optimize team efficiency and care delivery to Veterans. Provide health education and coaching on wellness, disease prevention, chronic care management, and self-management skills to Veterans and personal support person. Administering medications, immunizations and nursing procedures per established policies and guidelines. Utilizing all available tools, such as registries, to enable effective and efficient identification and intervention of individual patients and cohorts. Functioning at the full extent of the team member's relevant clinical privileges, credentials, scopes of practice, elements of practice, certification, functional statement, position description, or other VHA or local facility approved documentation of competency. Engaging patients in using health care, encouraging patients to engage in personal support persons, receiving input from patients and personal support persons regarding VA care. Providing evaluation and care consistent with licensure, certification, and functional statement with elements of practice, to patients assigned PACTs. Collaborating with PACT staff to develop comprehensive health care plans and care management plans for patients assigned to patient panels. Managing clinic workflow. Ensuring patients are placed in examination rooms in a timely manner and providing directions to patients as they move through the clinic environment. Perform other duties as assigned. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
